ACS Families: The lists below detail important forms that accompany enrollment, or are commonly used throughout the school year. Please find the sections below that pertain to the students from your family. The first section covers forms that are necessary for all students in Preschool through 8th grade. The other sections cover new families and students in various grades. Please make sure to carefully go through each list that pertains to your family. Forms for ALL families with students in Preschool - 8th Grade (WITH enrollment):
- Tuition Payment Auto-Deduction Form (for families who will utilize auto-debit)
- If you live in the Allendale Public School District and plan on riding the Allendale Public School bus, you must register for Allendale Public Busing
Forms specific to NEW ACS students:
- FERPA Form (Family Educational Rights and Privacy Act; due by July 15)
- Health Appraisal Form (due by July 15)
- Copy of Child's Birth Certificate
- Concussion information (read) and Concussion Acknowledgement Form (return to school for Y5-8 students)
Forms specific to Preschool through Kindergarten Students
- Health Appraisal Form (due by July 15)
- Copy of Child's Birth Certificate (if not already on file)
- Emergency Information Form (only preschool)
Form specific to Seventh Grade only
- FERPA Form (Family Educational Rights and Privacy Act; due by July 15)

Parent-Student Handbook
The ACS Parent-Student Handbook can be found here. This handbook provides information and procedures that are important to both parents and students at ACS. Important Notices:
- Safety Drill Record: Each year, schools in Michigan complete 10 emergency preparedness drills. This includes 5 fire drills, 2 severe weather drills, and 3 lockdown procedure drills. Schools are then mandated to post the drills (upon completion) in a place where they can be viewed online.
- Pesticide Application Notice
